Sandbanks Beach Volleyball Festival 2011 Entry Forms

Late entries arriving by email or in post will not be accepted

All entries must include payment

Entry fees will NOT be accepted at the event

The forms can be down loaded as a pdf Microsoft Word document. If you chose Word, you can complete the form on your computer, then save it, print off a copy, and then send to us with your entry fee.

You can check your entry on the Entry List page. Please allow a few days for us to update the list.

Parental Consent Form:	Download pdf	Must be completed by hand and signed - pdf Only
Each Junior Entry must include a completed Parental Consent Form

Please remember that last minute cancellations can cause event planning and organisational problems. If you cancel your entry on or after WEDNESDAY 6th July, we will not be able to refund your entry fee
